Privacy Notice for Patients
How I use your personal data:
Why does Gemma Webster Chiropractic collect information about you and how is this information used? Records are taken about your health in general, how you were after treatment and any treatment/advice you have received from me. These records are required to help provide you with the best possible care. I am obliged to seek signed consent from you before I can undergo any treatment. I collect relevant information about you to enable me to do my job effectively and safely. This can include medical history etc. I also have a legal requirement to keep treatment notes in accordance with the General Chiropractic Council (GCC) Codes of Practice. My records are electronic and on paper and I am working hard to upload all paper records onto Cliniko.(my booking system) All of your information is kept confidential and secure and only I have access to this. It is not shared unless obtaining consent by you using a medical release form. By booking online you agree to your data being stored within the Cliniko software.
Records which this clinic holds about you may include:
Address, telephone number and DOB
Previous medical history.
A history of past or present appointments
Your health records.
Details about your treatments or any advice given.
Relevant information from other health care professionals as given to me.
How do I maintain the confidentiality of your records

Here at Gemma Webster Chiropractic I have a legal obligation to keep information about you confidential. All of your information is held securely at the practice address and within ‘Cliniko’, a secure GDPR compliant password-protected cloud-based patient management system. Who do I share your information with?
I only ever pass on information about you to others, if there is a genuine need for it and you have given your FULL consent. This may be your GP, physio or any other health care professionals you require me to share data with.
I will not disclose any information about you to any third party without your permission or in case of a child’s information the parental consent, unless there are exceptional circumstances where the law requires information to be passed on.
Access to your personal information
You have a right under the General Data Protection Rules 2018 to request access to view or obtain copies of what information Gemma Webster Chiropractic holds about you and to have it amended should it be inaccurate. Your request must be made in writing to the clinic (details below). You may need to give proof of name (Photo ID) so that your identity can be verified. There is no charge for copies of your file, and I am required to respond to your request in normal circumstances within 1 month.
Retaining your personal data
Whilst you are a patient of mine I will continue to store and use your personal data. I have a legal requirement to keep your medical records for a minimum of 8 years from the date of your last appointment. After this time, I may continue to store your medical records indefinitely in order that I can provide you with the best possible care should you need any treatment in the future. You can request for your data to be removed from our system once the legally required 8 years has elapsed.
Objections/Complaints
Should you have any concerns about how your information is managed at Gemma Webster Chiropractic, please contact Gemma Webster Chiropractic first. Details of how to do this are mentioned below. If you are still unhappy following a review by the Data Controller, you can then complain to the Information Commissioner’s Office via their website (www.ico.org.uk).
Change of details
It is important that you tell me if any of your details such as your name or contact details have changed, or if any of your details I hold are incorrect.
